My fiancée celebrated her birthday yesterday. Because of my work, I couldn't travel down to her place (she stays in another state), but I called her severally.
I even sent her a gift through a courier service.
I was shocked when I woke this morning to receive series of messages on my device, saying now she knows I don't love her and all.
That I should have taken a day off to be with her...in short, she said she's not sure if she's willing to continue the relationship.
How do I handle this?

I don't see this as a very serious issue. She probably wants you to apologize for not being around on her birthday. Call and assure her that you love her and believe she will be ok.

Have you attended her birthday party before? Whether yes or no, apologies to her and give her an unscheduled visit.
